Your theory paper should include the following:
1)Create your own theory based on your specific practice area. My theory is – Chioma’s Theory of Health Literacy for Enhancing Self-care Abilities in Kidney Disease Patients
2) Include definitions of your theory and descriptions of at least two key concepts (My concepts are health literacy and self-care abilities) that make up your theory. Describe the relationships between these concepts.
3) Discuss the role research plays in the development and use of your theory in practice. In other words, why is this important? What is the literature saying about the topic you have chosen? (hint- literature review)
4) Provide a case example of how your concepts and theories would be used in a specific client situation in practice.
5) Describe the barriers to using this theory in practice.
The paper should have a title page, literature review, and references. No abstract. Running heads are required.
