Instructions
The QOL Model can be incorporated into the care of all patients with any diagnosis at any point during their illness.
- Develop a case study, of interest to you, discussing a patient/family/friend diagnosed with a chronic disease process (i.e. cardiac, renal, respiratory, etc), who would benefit from institute palliative care at the time of diagnosis.
- Incorporate the City of Hope/ELNEC QoL Model (introduced in Module 3 overview) as a tool for evaluating your patient. Provide explanations of the patient’s/family’s/friend’s perspectives of QoL related to the physical, psychological, social, and spiritual forces in their life.
- Discuss specific aspects of care (for each perspective-physical, psychological, social, and spiritual) that could be instituted for the patient/family/friend that would provide them with better QoL from their perspective (i.e. information related to disease process, ability to talk about their fears, adequate pain control, financial strain, etc).
- Discuss one additional outcome that you feel is important and should be attained.
- Support the information with the palliative care resources provided and at least two additional scholarly articles from the scholarly healthcare literature.
- Paper should be 3-5 pages not including title page and reference page. Follow APA formatting and the guidelines of the Written Assignment Rubric. Levels of headings are required.
This activity will be graded using the written assignment rubric.
Written Assignment Guidelines (APA 7th ed.)
Include the following APA (7th edition) formatting requirements for all written assignments:
- Title page
- Pagination (top right corner)
- Correct in-text citation format
- Correct reference format
- Margins
- Level Headings
- Page format
- 12-point font Times New Roman or 11-point font Calibri
- Double-space
