Assessment 3: Project Report – Health Promotion Activity

This assignment will address the following learning outcomes:
Critically review the role of the nurse in primary care settings within a primary health care framework (LO 1); Demonstrate knowledge and skills required to support vulnerable populations within a variety of primary health care settings (LO 2); and Investigate approaches used to work with the consumer and other health professionals to provide patient-centred care in primary care settings (LO 4).
Rationale:
This assignment will enable students to:
Develop skills in planning, developing and evaluating a health promotion activity based on a specific identified community need; and
Develop a deeper understanding of the role, scope and context of the practice of the nurse within primary care settings in an Australia community.
Instructions:
For this assessment, students are required to write a report incorporating the following tasks:
- Identify one of the specific groups that was identified in your Individual Presentation Assessment Item. Outline a new health promotion activity that aims to address the health needs of this group. 2. Describe the role of the nurse within a primary care setting relevant to your specific group (e.g. general practice setting OR general community health OR maternal and child health OR school health). Discuss how the eight principles of primary health care have been incorporated into your new health promotion activity

- At least ten references from credible resources must be used, mostly from journal articles; Make sure you use high quality sources of information, preferably from published and/or peer reviewed sources.
- Please note that Wikipedia, Better Health Channel, SCU study guides and the like are not appropriate sources of information;
- Significant marks will be deducted if inappropriate journal articles are chosen; Must have a separate page for the reference list;
- This report needs to be referenced using APA 6th Style.
- No cover sheet is required for this assessment item. Your report must include an introduction, body and conclusion;
- Use block paragraphs or have the first line of each paragraph indented – just be consistent; Your assignments must be submitted as either Word documents (with .doc extension, or .docx). PDF/Note pad assignments will not be accepted. No handwritten assignments will be accepted; Include either a Header or Footer with your name, student ID number and the page number;
- Use size 12 Times New Roman, Arial or Calibri font throughout the assignment; Use either 1.5 line spacing or Double spacing. You must use double spacing for the reference list as this is correct format for APA 6th edition referencing style; and
- Late assignments without an approved extension from the Unit Assessor will receive a penalty of -5% per day including weekends & holidays.
