Assignment guidance – as given to the students
The student will write a 2500 word discussion and reflection on their nursing decision-making to assess, plan and implement care related to an adult’s complex care needs that support them to live well with a long-term health condition. It will include the analysis and discussion of two care needs. The detailed evidence-informed care plan must be in an appendix (approx. 500 words) for these two care needs.

Introduction
- Define the long-term condition that the person has (this could include demographic data) and a brief biography of the service user (age, gender, the reason for the assessment, medical history, medications, etc.)
- what you will discuss, i.e. the specific sections/elements/themes, identifying the two care needs you will be exploring within your discussion ( see learning outcomes)
- how you will discuss, e.g. using a reflective cycle, professional experience, current literature, etc
- why you will discuss, e.g. importance of understanding the new role, professional accountability, etc
- Include a confidentiality statement to indicate that you are using a pseudonym for your service user in accordance with the NMC Code (2018)
LO1 Analyse assessment tools and findings to identify the diverse and complex psycho-social and physical care needs of an adult with a long-term health condition
- Analyse the nursing theory and assessment tool(s) used, e.g. Orem, RLT, Recovery Model, and other tools e.g. MUST, MMA, including your rationale for their use.
- Outline how the individual’s preferences, strengths, abilities, and needs were identified using a model/assessment tool
- Rationalise the selection of the two care needs, e.g. identify the impact that the needs were having on the person’s quality of life and activities of living (the impact could relate to pathophysiology/pharmacology), support with research and theory
LO2 Identify and appraise the evidence base supporting the complex nursing care of an adult with a long-term health condition
- Consider how management systems, e.g. Kaiser Permanente and House of Care and Government guidance e,g the Five Year Forward Plan and the NHS Long term Plan support your decision making
- Establish the importance of the care plan being evidence-based or evidence-informed
- Appraise the evidence base for the care plan (sources of evidence could include National Service Frameworks, NICE guidelines, professional literature, etc.), e.g. consider their relevance to your case study and how they support the clinical judgments you made. Consider the limitations of the evidence in relation to your individual
LO3 Formulate a person-centered, evidence-based, and sustainable care plan for an adult living with a long-term condition
- Evaluate the approach of formulating a care plan to support nursing a person with a long term condition
- Refer the reader to the appendix for your evidence-based care plan for the two needs identified
- Discuss the importance of person-centered, evidence-based/evidence-informed, and sustainable care, support with research.
LO4 Reflect on nursing decision making to support an adult to live well with a long-term condition
Use a reflective cycle and critically review the nursing decision-making to explore how the care plan will support a person to live well with a long-term condition. For example:
- How does practical, evidence-based care support an individual to live well with a long term condition
- What is the professional role and responsibility of a nurse working as part of the MDT to support a person to live well with a long-term condition?
- What challenges or barriers are experienced in delivering the planned care, e.g. resource limitations, poor interprofessional working, unclear roles, and responsibilities.
Appendix –The care plan for the two care needs must be in an appendix
Formulate a care plan of the 2 care needs that reflects the model of nursing used and presents:
- what the goals and interventions are (include references for the evidence used)
- identify short term and long term goals
- who is involved in achieving the goals and completing the interventions (the individual, healthcare professional(s), 3rd sector health charities/campaign groups)
- outline how the care needs will be evaluated
Ensure the care plan is person-centered, evidence-based/ evidence-informed and sustainable
Students may have more than one appendix to include examples of other tools or frameworks used such as MUST screening tool
