The Nurse Education Program at  xxxx reflects on the Nurse of the Future (NOF) Core Competencies: essential areas of expertise, based on a foundation of nursing knowledge, which professional nurses must acquire to meet the challenges and changes of future nursing practice. The purpose of this assignment is to demonstrate how these essential concepts of nursing practice were applied in the care of a patient you encountered this semester in the clinical setting. A group assignment in the Leadership clinical in NUR 4 provides the opportunity to demonstrate the competencies of Leadership, Quality Improvement, Professionalism and Systems-Based Practice through the design of a Quality Improvement Project in the clinical setting.

Section 1

Describe a patient you cared for in the clinical setting. Include the patient’s age, sex, employment, language, culture and family status. Summarize the patient’s presenting problem, the course of their hospitalization, and the major nursing and interdisciplinary treatments obtained. Include a brief summary of the patient’s history, including medical, mental health or social history that affected their current illness. Include a summary of the patient’s discharge plan.  DO NOT include any potentially identifying patient information including: name, date of birth, dates of hospitalization or name of health care facility.

Section 2

Patient-centered care is defined by NOF as:

Provision of holistic care that recognizes an individual’s preferences, values, and needs and respects the patient or designee as a full partner in providing compassionate, coordinated, age and culturally appropriate, safe and effective care.

Provide examples of how patient-centered care was provided to your patient. Areas to address include: cultural and religious preferences, physical comfort and emotional support, involvement of the patient and family in the plan of care, barriers to the involvement of the patient or family, ethical concerns, any conflicts and how conflicts were resolved.

Section 3

Teamwork and Collaboration is defined by NOF as:

Function effectively within nursing and interdisciplinary teams, fostering open communication, mutual respect, shared decision making, team learning, and development.

Analyze your functioning as a member of the health care team. Briefly describe the roles of other health care team members who were involved in your patient’s care. What were the contributions of other disciplines? Did you note differences in patient interaction or communication with patients? How were shared goals accomplished? Were there different opinions among team members concerning the patient’s care? If noted, how were differences resolved?

Communication is defined by NOF as:

Interact effectively with patients, families, and colleagues, fostering mutual respect and shared decision making, to enhance patient satisfaction and health outcomes.

Describe therapeutic techniques you used to communicate effectively with your patient. Provide examples of adaptations you made based on your patient’s preferences and response. How was non-verbal communication used by you or your patient? Evaluate how effectively essential information was communicated to your patient. What different methods and styles of communication did you employ when communicating with colleagues and other members of the health care team?

Section 5

Safety is defined by NOF as:

Minimize risk of harm to patients and providers through both system effectiveness and individual performance.

Assess the systems in place to reduce the risk of harm to your patient. What specific actions did you take to ensure your patient’s safety? Did you identify any potential hazards to your patient? What systems measures were in place to ensure staff safety? In your analysis, include National Patient Safety Goals to: ensure patient identification, improve staff communication, use medication safely, use alarms safely, prevent infection and identify patient safety risks.

Section 6

Informatics and Technology is defined by NOF as:

Use advanced technology to analyze and synthesize information and collaborate to make critical decisions that optimize patient outcomes.

Demonstrate how information and technology systems impact patient care in the clinical facility. Include specific examples of how you used information and technology to provide care. In addition to benefits of information and technology, address any concerns or potential problems you see with expanded use of information and technology.

Section 7

Evidenced-Based Practice is defined by NOF as:

Identify, evaluate, and use the best current evidence, coupled with clinical expertise and consideration of patients’ preferences, experience and values to make practice decisions.

Choose one evidence-based article from a peer-reviewed nursing journal published within the past five years that addresses one of the Nurse of the Future (NOF) competencies you applied to the care of your patient. Contrast the main points in the article with your actual patient care experience. Does the article differ from or confirm your experience? Upon reflection, is there anything that you would do differently? Summarize why and how NOF competencies are important to professional growth.
