Family Transitions – Alternate Gerontology Assignment
Standardized tools are an efficient and effective way to collect and organize data from the assessment of an older adult. This alternate assignment has been developed with the understanding that you are unable to complete a functional assessment of a client, as you are not currently in a clinical placement.
Process
You will identify a relevant health issue pertaining to Older Adults. You will choose a standardized tool that could be used to perform a detailed assessment of that issue (For example; Urinary Incontinence: CHAMMP Tool). You will be expected to consider how the tool could be applied in practice, and provide analysis of your nursing assessments and interventions.
Paper Guidelines
The body of your paper will be a maximum3-4 pages in length, excluding title page, references &/or appendices. It will consist of:
- A brief introduction to the standardized tool that you would use to perform a focused assessment and approach related to the issue (Paragraph form with evidence)
- A description of how you would perform an assessment of a client, using this tool (Point form).
- Identify and describe nursing interventions to help the client in resolving or coping with the identified problem. These interventions will be based on a minimum of 3 relevant and current RNAO BPG, scholarly articles or course text references. These resources should be used to support your interventions. All references must be imbedded into the paper. (Paragraph form with evidence)
- Describe how you would evaluation your selected interventions (Point form)
- You will conclude your paper with why this assessment tool is valuable to your practice (Point form).
Your paper will be APA format. (Please review the APA Guide from Durham College for how to properly format your paper including appendices).
Your paper must be submitted at the identified due date and time in week 5. All documents must be written in Microsoft WORD format and will automatically be sent to Turnitin for originality verification upon submission.
All requirements for the paper are outlined in the rubric provided.

Introduction:
Provide a description of the relevant health issue pertaining to older adults, as well as an introduction of the assessment tool used to perform a focused assessment.
Assess:
Provide a detailed description of how you would perform an assessment of a client using this tool.
Plan:
Identify what factors you would consider when planning your interventions to address this problem.
Intervention:
Provide 3 personalized and applicable interventions to address the area of concern. Back up your recommended interventions with evidence (from 3 different sources – only 1 can be an RNAO document)
Evaluation:
Include how you would evaluate the outcomes of your 3 interventions.
Application:
Discuss why this tool is valuable to your practice as a student.
