Sally is a 56 y/o female who presents to your office complaining of depression. She has been hospitalized for depression and suicidal thoughts on 3 occasions, once in her 20’s, once after the birth of her daughter when she was 31, and again about 5 years ago. She is tearful in the appointment stating she doesn’t want to go back to the hospital, but she just knows she is heading down a dark path. She is currently prescribed Zoloft 100mg 2 pills daily for depression and Xanax 0.5mg BID PRN for anxiety. She reports having tried Cymbalta (made things worse), Prozac (helped for a while and stopped), Lexapro (no response), Pristiq (caused agitation).
