Client Profile:
The nurse is caring for a 50 year old male who has recently arrived to the ward with congested heart failure(CHF). Mr Sartaj Manjari is of Indian descent and has lived in Australia for 20 years.He is the owner of a local popular Indian restaurant and lives with his wife and 3 children.While conducting an holistic nursing assessment on admission, the nurse is aware that certain aspects of cardiovascular, peripheral vascular, integumentary and respiratory assessments are a priority responsibility in performing an episodic or problem-centred database approach on this patient.
Assessment task:
Critically discuss the nursing responsibilities in undertaking an initial cardio-respiratory assessment of this man including relevant aspects of health history taking and physical examination specific to his presenting condition of CHF.
Include in the discussion the assessment approach and techniques that the nurse would utilise for this particular patient, paying particular attention to:
*interviewing/communication skills required by the nurse
*demonstrating a planned and organised approach in conducting a holistic nursing assessment including the relevant aspects of subjective(chest pain,shortness of breath,dyspnoea,cough,fatigue,cyanosis or pallor,palpations,peripheral oedema) and objective(vital signs including perfusion assessment,auscultation of heart and breath sounds) data to be gathered
*any developmental,psychosocial,cultural,spiritual and/or mental health considerations that should be noted when undertaking the health assessment.
Furthermore, consider the ‘What'(what specific aspects of data the nurse needs to gather?), ‘Why’critically analyse(why the nurse should gather this specific data) and ‘How'(how the nurse would undertake the above given the special considerations of the holistic domain).
