You will develop a concept map of a health promotion topic and the factors to consider when implementing health promotion strategies for your topic.  To demonstrate your knowledge of the content in your concept map, you will provide a 2 page summary report explaining each section.  Your report will be 3 pages in length, not including your title page and reference page. 

Please complete the following sections for your concept map, and follow the instructions on how to complete your 2-page summary.  Please pay attention to the required references for each section.

Section 1:

Identify a health promotion topic that is of interest to you. (eg: nutrition, clean water, vaccine use… etc).  Identify 3 key concepts related to the topic in order to demonstrate your knowledge of the topic.  (this should be box #1 in your concept map).  Write a paragraph expanding on these key concepts to demonstrate your knowledge of the topic using at least 1 evidenced based reference.

Section 2:

Relate your topic to a target cultural population (box #2).  Explore the issues related to your topic from the perspective of your selected cultural group Demonstrate your cultural knowledge by identifying 3 key issues that you would consider when addressing the health promotion topic.  These three key factors should be included as box #2a, 2b & 2c in your concept map).   Please write a paragraph describing your awareness of the cultural factors that influence your selected health promotion topic. Please include at least 1 evidenced based reference.

Section 3:

Identify 2 Social Determinants of Health (SDoH) that relate to your topic (Box 3). Briefly describe 2 ways each SDoH impact the health promotion topic (Box 3a & 3b).  Include these as box #3 in your concept map.  Provide a paragraph on how the SDoH’s impact your health promotion topic and their influence on the target population.  Please include at least 2 evidenced based references.

Section 4:

Identify 3 health promotion strategies you would use to address the health promotion topic with your target population (box 4).  Please list and describe these health promotion strategies in box #4a, 4b & 4c on your concept map.  Provide a paragraph explaining your health promotion strategies and consider teaching and learning principles you would use to assist in implementing your strategies.  Please include at least 2 evidenced based references.
