Identification of nursing Problem
Centers for Medicare and Medicaid Services (CMS) indicated one in five patients readmitted from hospital discharge in 2015. According to Agency for Healthcare Research and Quality (AHRQ) (2019), the hospital readmission rate is one of quality indicators that implicates
the level of clinical performance and outcomes of healthcare organizations. High readmission rate will be associated with low healthcare service quality. Currently, every healthcare system in United States must comply with the government’s rating system such as the Hospital Consumer Assessment of Healthcare Providers and Systems Survey (HCAHPS) score which indicates the patient satisfaction score. Poor reputations with HCAPHPS in healthcare industry causes
the organization’s financial jeopardy. The Hospital Readmission Reduction Program (HRRP) was initiated to reduce avoidable readmissions and this program penalizes hospitals up to three percent of Medicare reimbursement for “excess” readmissions. Seventy five percent of hospitals in U.S. were penalized in fiscal year 2015 and it was worth an estimated over four hundred million dollars (Caracciolo et al., 2017).
Unexpected readmission after discharge could cause low quality of life for patients and families because of delay in going back to normal life. There are two major factors affect to avoidable readmission: Discharge planning and patient self- care education.
